Cluster B Personality Disorders
Borderline, Narcissistic, Histrionic, and Antisocial — the ‘dramatic, emotional, or erratic’ cluster — with evidence-based treatments for most.
Cluster B personality disorders share themes of emotional dysregulation, impulsivity, and interpersonal instability. They are the most common cluster — and BPD has the strongest evidence-based treatments of any personality disorder.
The Cluster B disorders — Borderline, Narcissistic, Histrionic, and Antisocial — are the most studied. Borderline Personality Disorder in particular has multiple evidence-based treatments (DBT, MBT, TFP, Schema Therapy) that produce meaningful, lasting improvement.
The Four Cluster B Disorders
Borderline Personality Disorder (BPD) — emotional dysregulation, unstable relationships, identity disturbance, impulsivity, self-harm.
Narcissistic Personality Disorder (NPD) — grandiosity, need for admiration, lack of empathy.
Histrionic Personality Disorder — excessive emotionality and attention-seeking.
Antisocial Personality Disorder (ASPD) — pervasive pattern of disregard for and violation of others’ rights.
Evidence-Based Treatment
BPD has four evidence-based treatments: Dialectical Behavior Therapy (DBT), Mentalization-Based Treatment (MBT), Transference-Focused Psychotherapy (TFP), and Schema Therapy. All produce meaningful, lasting improvement.
NPD responds to TFP and Schema Therapy in structured settings. Histrionic PD responds to psychodynamic and cognitive approaches. ASPD remains challenging but cognitive remediation and contingency management in residential settings produce meaningful gains.
Frequently Asked Questions
Are Cluster B disorders treatable?
BPD has multiple highly effective evidence-based treatments. NPD and Histrionic PD respond meaningfully to structured psychotherapy. ASPD is the most challenging but improvement is possible in residential settings.
Why does Cluster B get the most attention?
Cluster B disorders cause the most acute distress and clinical contact — BPD in particular accounts for a large share of psychiatric ER visits. This volume has driven the most research and the strongest evidence base.
