Which Aetna Plans Cover Out-of-Network Rehab?
Open Choice PPO and Managed Choice POS cover out-of-network rehab. HMO, Aetna Select and Elect Choice EPO generally do not. Verify your plan free in 15 minutes.
Your Plan Type Decides Almost Everything
Whether Aetna covers rehab is rarely the real question — under federal parity it generally does. The question that actually determines your options is which Aetna plan you hold, because that decides whether you can use a program that is not in Aetna’s network.

Aetna Plan Families
Aetna organizes its commercial plans into families, and the names on your card map to real differences in access:
- Aetna Standard Family — the core plans, ranging from Open Choice® PPO, which requires no referrals and no primary care physician selection, through to HMO and exclusive provider plans where a PCP guides specialist care.
- Aetna Open Access® Family — similar benefits, but members are encouraged rather than required to pick a PCP, and can go directly to network specialists without a referral. Includes Open Access Managed Choice® (POS), Open Access Elect Choice (EPO) and Open Access HMO.
- Aetna Select℠ — member chooses a PCP, and a PCP referral is generally required for specialist care. Outside emergency or out-of-area urgent care, benefits are not covered outside the network.
- Aetna HealthFund® — a deductible-based plan paired with an employer-established health fund, combining referral-free network access with the ability to seek out-of-network care.
- Aetna Medicare℠ — available as HMO, PPO, and the Aetna Medicare Open℠ private-fee-for-service plan.
In Washington, the product referred to as HMO is called Primary Choice℠ and is offered by Aetna Health Inc.
Which Plans Cover Out-of-Network Rehab
Generally yes: Open Choice PPO, Open Access Managed Choice POS, HealthFund plans, and Aetna Medicare PPO. You can use a program outside the network at a higher share of cost, which is what makes destination and specialty treatment possible.
Generally no: HMO, Open Access HMO, Aetna Select and Open Access Elect Choice EPO. Except for emergency or out-of-area urgent care, benefits are not covered outside the network.
The National Advantage™ Program — Worth Asking About
This is the detail most people never hear, and it can materially change what you pay.
Aetna’s National Advantage Program provides access to contracted rates for claims that would otherwise be paid as billed — including the out-of-network portion of managed care plans and indemnity claims. The network includes providers Aetna contracts with directly plus others reached through vendor arrangements.
In practice: a rehab program that shows as out-of-network may still be reachable at a negotiated rate through NAP, which can reduce your bill substantially. Ask the admissions team specifically whether they participate through the National Advantage Program — it is not a question most people know to ask.
Two honest caveats. Aetna states it does not credential, monitor or oversee providers who participate through third-party contracts, and because several factors determine whether a discount applies, no particular level of discount is guaranteed. Treat NAP as worth checking, not as a certainty.
Aetna Signature Administrators®
If your card mentions Signature Administrators, your employer is almost certainly self-funding the plan and renting Aetna’s network to price claims. Three consequences:
- Your benefits come from the employer’s plan document, not an Aetna insurance policy
- The provider directory is separate from the main Aetna one — ask the facility to check it by name
- Appeals run under federal ERISA rules, so complaints go to the US Department of Labor rather than your state insurance regulator
Behavioral Health Access Is Different
One thing that works in your favor across plan types: the Aetna Behavioral Health Program lets members access covered behavioral health services directly, without a referral from a primary care physician — even on plans where referrals are required for other specialist care. Prior authorization for higher levels of care still applies, but you do not need to route through a PCP to start.
Check Your Plan Is Still Active
If your Aetna plan came from the Health Insurance Marketplace, it ended on December 31, 2025. CVS Health withdrew Aetna from the ACA individual market for the 2026 plan year across all seventeen states where it sold those plans. Employer coverage, Medicare Advantage and Medicaid plans were not affected.
Confirm which you hold before a facility runs your benefits. A verification against a terminated policy comes back as no coverage, which is not the same thing as being uninsurable.
Verify Your Aetna PPO rehab Benefits
Free, confidential Aetna benefits verification.
We will confirm whether your Aetna plan is active, what it covers, which programs are in network, and what you would owe. If your plan came from the Marketplace and ended in January 2026, we will tell you that plainly before you go further. About 15 minutes, no obligation.
