Does Aetna Cover Residential Rehab?
Aetna covers residential and inpatient rehab when ASAM medical-necessity criteria are met. Authorization comes in increments with continued-stay review. Verify free.
Aetna Residential Rehab Coverage
Yes. Residential treatment — ASAM Level 3.5 clinically managed, or 3.7 medically monitored — is a covered benefit when criteria are met. It is also the level of care where authorization is most actively managed, so it helps to understand how the decision is actually made.

How Aetna Decides Residential Is Medically Necessary
The question is not how severe the addiction feels. It is whether a lower level of care could reasonably work. Reviewers generally look for:
- Withdrawal risk requiring 24-hour monitoring
- Co-occurring medical or psychiatric conditions complicating treatment
- Prior outpatient or IOP episodes that did not hold
- A home environment that makes outpatient recovery unrealistic
- Inability to maintain abstinence long enough to engage in treatment
Anything documented in that list strengthens the case. It is worth telling the assessing clinician about failed prior attempts rather than presenting the best version of your history — here, the difficult details are the ones that support coverage.
Concurrent Review and Continued Stay
Aetna does not typically approve a whole residential stay at admission. It authorizes an initial period, then reviews at intervals — commonly every 5–7 days — with extensions based on documented progress and continuing need.
This is standard across the industry and is not a sign the plan is looking for a reason to stop paying. The program’s utilization-review team manages these calls, and their quality varies considerably. It is a fair question to ask before admission: who handles continued-stay reviews, and how often do they win extensions?
What Your Plan Type Changes
Open Choice PPO and Open Access Managed Choice POS plans can use out-of-network residential programs at a higher cost share. HMO, Aetna Select and Open Access Elect Choice EPO plans generally cover in-network facilities only, except in an emergency.
If your plan is administered through Aetna Signature Administrators, your employer is self-funding and the plan document sets the rules — check it rather than assuming standard terms. See Aetna PPO rehab benefits.
If Residential Is Denied
Appeal. Internal appeal first, handled by the program’s review team; then external review by an independent reviewer who is not Aetna; then a complaint to your state insurance regulator, or to the US Department of Labor if the plan is self-funded.
Denials are frequently overturned, particularly where the initial submission was thin on documented prior treatment attempts.
Check Your Plan Is Still Active
If your Aetna plan came from the Health Insurance Marketplace, it ended on December 31, 2025. CVS Health withdrew Aetna from the ACA individual market for the 2026 plan year across all seventeen states where it sold those plans. Employer coverage, Medicare Advantage and Medicaid plans were not affected.
Confirm which you hold before a facility runs your benefits. A verification against a terminated policy comes back as no coverage, which is not the same thing as being uninsurable.

Frequently Asked Questions
How many days of residential rehab will Aetna cover?
There is no fixed number under parity. Coverage continues while medical necessity is documented, reviewed in increments. Some employer plans impose their own annual day maximums — verify yours.
Does Aetna cover out-of-state residential rehab?
PPO and POS plans generally can. HMO, Aetna Select and EPO plans generally cannot, except in an emergency.
What if Aetna denies residential treatment?
Appeal internally, then request external review by an independent reviewer, then escalate to your state regulator or the Department of Labor for self-funded plans.
Do I need a PCP referral for residential rehab?
No. The Aetna Behavioral Health Program allows direct access to covered behavioral health services without a PCP referral, though prior authorization from the facility is still required.
